[Help] I lost my dog's vaccination papers and I need to take him into Canada from the U.S. He has his tags, but how can I get the papers?

Long story, but my ex got my dog vaccinated at her place of work which is a humane society (not a traditional vet). She never gave me the papers, but my dog is properly tagged. Is there a way for me to get his vaccination certificate from the state (I live in Illinois) by presenting his tag information? I'm worried about contacting her place of work for the certificate because we had a legal battle over the dog (which has since been settled, but is still problematic in our relationship now).Any advice would be helpful.
