[Help] Why is my dogs Vulva so big?

So I've had my dog for about 2 years now, she is a rescue and I have no idea about her past. I never really noticed before how large her Vulva is and how it protrudes from her body and she also has large nipples. Not sure if anything is medically wrong or maybe she was a possible breeder? She has never had difficulties during urination and doesn't seem that it's painful for her but she spends a lot of time cleaning her Vulva and Anus to the point that there's barely any hair left on her Vaginal area. Whenever I bathe her I always try to clean those areas non-invasively, I've also been having a hell of a time with her breath having an abnormally fishy oder.
